| Ongoing Care for CMAP Free Clinic Patients with Chronic Illnesses | ||||
| Treatment prescribed for patients with chronic illnesses often is complicated and can be confusing, especially to elderly individuals. Our registered nurses who serve as case managers, coordinate, counsel and monitor the course of treatment prescribed by a physician and provide a link between our clinics, MedBank and other services a patient may need. The Chronic Disease Management program provides patients with the ongoing support and counseling they need to adhere to healthy lifestyle changes and avoid unnecessary medical emergencies that often result in hospitalization. In addition to the services provided by the volunteer doctors and clinic nurses, CMAP's diabetic education program leader and certified diabetic educators work with approximately 40 diabetics. The volunteer team teaches free classes and works one-on-one to help patients set achievable goals and monitor progress. Our volunteer pharmacist also plays a vital role in this program along with CMAP volunteer physicians. With this level of care and support, our patients have been able to accomplish more in one year, than many people accomplish in a decade: discovering the way to living a healthier, longer life. In addition to diabetes education classes, the Brunswick clinic offers free women’s health clinics and smoking cessation classes to help smokers kick the habit for good. |
