    MEDBANK - PHARMACEUTICAL ASSISTANCE PROGRAM    
         
    Access to medications is the number one healthcare need in America. CMAP's staff helps guide MedBank members through the complex steps required to access free medication from pharmaceutical companies. This assistance provides many chronically ill patients with medications that would be otherwise unaffordable.

Since May 2002, CMAP has helped more than 3,032 chronically ill individuals receive over 22,846 prescriptions. As a result, we have distributed more than $13.1 million in free medications in five years.

    ELIGIBILITY:    
    You may qualify for pharmaceutical assistance IF . . .    
   
  1. You do NOT have prescription drug coverage.
  2. Your health insurance does NOT cover medications.
  3. Your doctor signs up to participate in the CMAP MedBank Program.
  4. You receive Medicare benefits but do NOT qualify for extra help through Medicare Part D, or -
  5. You receive Medicare benefits but did NOT enroll in a Medicare Part D plan.
   
         
    You will not qualify for pharmaceutical assistance IF . . .    
   
  1. You do not have a doctor who you see on a regular basis.
  2. You do not have a doctor who participates in the MedBank program. (Doctors in the emergency room at the hospital or those at the immediate care centers do not qualify.)
  3. You have any type of drug coverage including health insurance, Medicaid and some drug discount card programs.
  4. You receive Medicare benefits and have reached the coverage gap in your Medicare Part D plan.
   
         
   
   
         
    FREQUENTLY ASKED QUESTIONS:    
         
    Q: When are the MedBank offices open?    
    A: Monday - Friday from 8:30 a.m. to 5:00 p.m.    
         
    Q: Do I need an appointment?    
    A: Yes. Patients are seen by appointment only.    
         
    Q: What do I need to bring with me to the appointment?    
   
  1. A Medicaid denial letter.
  2. Your federal tax return (the original 1040 form with the profit and loss statement)
  3. Paycheck stubs for one month (if paid weekly you'll need 4 stubs, if paid bi-weekly you'll need 2 stubs)
  4. Letters from family members or friends who may now be helping you pay bills (food, housing, utilities, etc.)
  5. Other statements that document benefits such as food stamps, Social Security, AFDC, unemployment, annuities, retirement, VA benefits, or child support
   
         
    Q: Are there any costs involved?    
    A: Yes. There is a small, monthly membership fee to cover the cost of processing applications. This fee is based on your monthly income.    
         
    Q: What pharmaceuticals are covered?    
    A: Most medications generally used to treat chronic illnesses.
