An Overview

In 2001, Coastal Georgia leaders from Camden, Glynn and McIntosh counties became concerned about the growing problem of healthcare access for residents. Recognizing this need, they successfully competed for an Access Georgia Rural Health Matching Grant and subsequently formed the Coastal Medical Access Project (CMAP).

CMAP was formalized as a nonprofit in February, 2002. Over the past few years, CMAP developed an innovative approach to meet healthcare by providing three distinct but coordinated services for those living in Camden, Glynn and McIntosh counties. These include primary care, free pharmaceuticals, and the management of care for people living with a chronic illness such as diabetes or heart disease.


CMAP
's mission
is to help build healthier communities focusing primarily on Coastal Georgia through programs that increase access to healthcare. This is accomplished through collaboration among the medical community, faith-based organizations, local businesses and volunteers.


CMAP serves residents of Coastal Georgia who are uninsured or underinsured. Approximately 24,000 people in Camden, Glynn and McIntosh counties who do not have health insurance and thousands more under-insured individuals may be helped by CMAP programs.

These folks are not on Medicaid or Medicare. They are working usually 2-3 jobs and none of their employers offer health insurance. We also serve people who are retired, have worked all their lives but are not quite old enough for Medicare. These people are our friends, neighbors, relatives, and employees. They are the people who mow our lawns, wait on our tables, and look just like us. In other words, CMAP serves our community.

The Problem: The combined population of Camden, Glynn and McIntosh counties is over 128,000 people. The 2003 percent of population living below the poverty level ranged between 10.8 in Camden and 17.4 in McIntosh. CMAP Volunteer Physicians are generous- more than 70 Volunteer MDs serve our patients. However, too often uninsured and underinsured residents use the hospital emergency department for non-emergency services. This overwhelms hospital emergency services and is costly.

The Solution: CMAP leader partnerships formed with pastors and their faith communities, housing authority officials, service organizations, private businesses, the Southeast Georgia Health System, Coastal Health District, Georgia's Department of Family and Children's Services, school systems, and volunteers to assist in the planning, design, and implementation of CMAP services. Together we provide:

Clinic Services including primary, specialty, dental and vision care:

  • 2 Free Clinics
  • Specialty Referrals (approximately 1,889 from 2/2003 thru 6/2009)
  • Volunteers:
    • 70 Physicians
    • 5 Physician Assistants and Nurse Practitioners
    • 37 Nurses
    • 4 Pharmacists
    • 41 NonClinical/Administrative

MedBank Prescription Assistance (2002 thru 6/2009):

  • 30,289 Prescriptions filled
  • > $16.9 Million
  • 102 Pharmaceutical Companies

Alive & Lively - Chronic Disease Prevention and Management (CDPM):

  • RN's Provide:
    • Clinical Support
    • Education, and
    • Outreach
  • Referrals for:
    • Financial
    • Social, and
    • Medical Services
The CMAP network includes the public health district's key leadership - Southeast Georgia Health System's Brunswick and Camden Campuses, physicians, schools, local government, county commissioners, the Kings Bay Submarine Base, local state and national funders, and partner at the Colleges and Universities.
