    SERVICES PROVIDED    
         
    CMAP provides three distinct but coordinated services in southeast Georgia. Even though each service has its own criteria, sometimes these three services can overlap within CMAP if a patient meets each separate set of criteria.    
         
    Henri C. Woodman Community Health Clinics    
   
  • Two free clinics, one located in Brunswick GA and one located in
    St. Marys GA, provide primary care by appointment only.
  • Clinics are staffed by volunteer physicians, nurses, pharmacists, and lay volunteers.
  • Over 9,394 patient visits have been provided at CMAP's free clinics since February 2003.
  • 75% of these visits were "Emergency Room diversions".
  • 70 physicians, 5 Physician Assistants and Nurse Practitioners, 37 nurses, 4 Pharmacists and 41 administrative volunteers provided over 3,429 volunteer hours in 2007.
   
         
    MedBank    
   
  • Patients are guided individually through the complex process of obtaining free or low cost prescription medication from pharmaceutical companies.
  • More than $13.1 million (retail value) in medications have been accessed.
  • 22,846 prescriptions have been filled since MedBank was established.
  • 3,032 patients have been assisted.
   
         
    Chronic Disease Management    
    Nearly 100% of our patients received Chronic Disease Management services. Registered nurses serve as case managers to help patients with chronic illnesses follow a prescribed course of treatment. Volunteers teach classes and work one-on-one to help patients set achievable goals and monitor progress.
